EDEN Essentials

 

Eden Essentials for social prescribing, link workers and allied roles

Eden Essentials is a new programme that aims to provide people working in many different health, social care and wellbeing roles, with essential knowledge to care for or support people living with diabetes.

Eden Essentials consists of three elements:

Eden Essentials

A one hour recorded video lesson that can be viewed at a time convenient to the delegate. This lesson provides an accurate and up to date overview of diabetes that is evidence and research based, in an easily digestible style. The lesson covers:

  • What diabetes is and the different types of diabetes

  • Risk factors for type 2 Diabetes and how to prevent it

  • The possible symptoms and consequences of diabetes

  • What steps can be taken to ensure a positive outlook for those living with diabetes

  • Healthy lifestyles and reducing risk for people living with diabetes

Eden Elect

A live webinar using the Eden Elect educational tool. This session is highly interactive and is a valuable time for delegates to discuss diabetes and general health topics that are pertinent to their role, with experienced healthcare professionals.

We will use Eden Elect as a teaching aid and discussion prompt. The real benefit of this, being that the delegates can ask questions and discuss whatever areas they feel that they need to increase their confidence in.

Eden Elect uses a digital ‘body’ to provide the essentials on particular subjects quickly. If more depth of knowledge is required, the user has the option to read further on each body part or lifestyle image. This tool saves time by providing all you need to know in one place and delegates will have continued access to this resource.

Eden Elect is available for you to use independently as a diabetes decision-making resource.

  • Decision aids

  • Useful links for further information, websites and infographics

  • How and where to make relevant referrals for patients

In one place, this gives the right information at the right time for the right person and situation. It increases knowledge and confidence and ensures appropriate signposting and referral is completed, saving resource and improving efficiency.

Eden Essentials has been designed for the following client groups:

  • Social Prescribers and Link Workers

  • All other non-clinical, patient facing primary care roles

  • People working in lifestyle and prevention services, i.e sport and leisure, nutrition, weight management, social service and mental health/ advocacy roles

 

Why has Eden developed this training package?

By equipping healthcare professionals and staff with knowledge and confidence to provide excellent diabetes care or appropriate signposting, the benefits are passed on to the patient.

The ultimate goal of the Eden team is to improve the quality and longevity of life of people living with diabetes.

Primary care workload stresses, especially during and after the global pandemic, can be somewhat relieved by utilising every member of staff in every role available. We aim to assist staff members in associated roles, thereby enabling them to alleviate pressures on GP’s, practice nurses and clinical pharmacists.
